I understand that pets act differently at boarding facilities than they do at home and that puts my pets at risk for picking on each other if they are kept in the same suite (kennel) during their boarding stay at Creekside Veterinary Hospital. *

I understand and accept.

I understand that when pets pick on each other, injuries from attack can vary from relatively minor to life-threateningly severe. *

I understand and accept.

I understand that Creekside Veterinary Hospital’s team is not on site 24/7 and cannot observe your pet to break up instances on a 24/7 basis. *

I understand and accept.

I understand that in the case that one or more of my pets has suffered injury as a result of being housed together, I will be responsible in full for the cost of medical care which could include: after hours call in fees, surgeries, oral medication, humane euthanasia, etc. *

I understand and accept.

As with any other medical service, payment is due in full at the time of services. In the event that your pet is in need of medical care and we cannot reach you, you release authority to the attending veterinarian to provide care and will agree to pay for the services (up to and including euthanasia if deemed necessary due to suffering and/or poor return to quality of life) in full prior to discharge from the hospital. *
